To make an appointment, first ensure you have a valid referral from your Doctor, Dentist or Specialist. 

 

Please bring the following:

• Referral letters

• Your valid Medicare Card (hard copy required)

• Private Health Insurance details

 

Your first appointment will be a consultation only. Your Surgeon will obtain all the necessary history and conduct an examination if required. 

Further investigations may also be necessary.

A Cone Beam CT scan will be done upon arrival. Other scans and blood tests may also be ordered by the Surgeon. 

Your Surgeon will discuss a treatment plan with you, which on occasion may include Surgery.

Our receptionist staff will email or text you a link to our Patient Details & Medical History form, please complete this prior to the day of consultation to allow us to prepare for your arrival.
